• Form Submissions: By submitting your contact details, insurance information, or pre-admission forms through the Site, you represent that all information provided is accurate and truthful. 
• No Clinician-Patient Relationship: Initiating contact, calling an intake helpline listed on this site, or submitting an online form does not establish a formal clinician-patient, doctor-patient, or therapeutic relationship between you and Akasha Treatment Center, Inc. A formal relationship is only established upon completing an official, in-person clinical intake and signing physical admission agreements at our facility.
